FORMS REQUIRED
Based on the work and material to be handled, specific Risk assessment forms will be filled and submitted two weeks before the next IBC meeting to ibcbox@cgiar.org.
EOHS will review the risk assessment form and determine whether it proceeds to IBC or will be reviewed by EOHS and an approval given for project to commence at EOHS level.

RA1.2 – hazardous substances risk assessment form.
RA2.1 – fieldwork risk assessment form.
RA3.1 - risk assessment for biological agents.
RA4.1 - risk assessment for recombinant DNA.
RA4.2 - risk assessment for retroviral vectors.
RA4.3 - risk assessment for modified prokaryotic and/or eukaryotic cells.
RA5.1 - risk assessment for genetically modified animals.
RA6.1 - risk assessment for genetically modified plants.

CONTAINED USE APPLICATION FORMS
In addition to risk assessment forms for GM animals or plants scientists will be required to fill the following forms for contained use of genetically modified plants or animals before submission to National Biosafety Authority (NBA) are as follows
I.    Application form for Contained use activity Laboratories, Green houses and Growth chambers (Plants)
II.   Application form for contained and confined use/trials (genetically modified animals and animal health inputs)
III.  Application form for Confined field trials for plants.
 

APPLICATION FEES
For applications that will be submitted to National Biosafety Authority for contained use activities, the following payment will be required before an application can be reviewed and approved by NBA.

Cheques will be payable to National Biosafety Authority (NBA)
